HEllo friends,
I have aplied for a new adsense account in my mothers name.My account is not active & they have replied me with following message.

Account Not Active
An AdSense account does not exist for this login, as it is associated with an unapproved application. For more information about your application, please review the message we sent to the email address you provided with your application.

Return to AdSense home.

Please suggest me suitable appeal form.(The form contains email id & publisher name. I have lost the link. Please help m with the link)
