It was meant to be that way even before panda and penguin . Its one of the reasons that meta keywords were devalued by google. You're right about content and such, but you also need to be careful with metadetails, page titles etc.

keyword stuffing has always been gone. Its one of the reasons that google devalued meta keywords. from what I have heard 1 - 3 is good. As long as it reads naturally, and it does not seem like you are trying to over promote a product.

Yeap, I dont see the point in counting your exact keyword density. It is better to just write something natural. That way your readers will like it and come back for more. All you have to do is check you have the keyword dotted around the text, in a natural way. Google will pick up on it if it isn't natural (I think)

Yes you are right because it can hurt our website ranking badly because google consider it as black hat technique. When we talk about black hat technique we should avoid all kinds of black hat techniques such as cloaking, keywords stuffing, dorway pages, copied content, low quality backlinks etc.
