My questions:

I like that you say the forums will be active ones, but why are the forums to be all new? What's wrong with established forums if they are active? By any chance, do you create the forums, or something?

You say you never post signature links, but would you consider posting them if the client requests (and gives them to you)? When a forum allows sig links and most members are utilizing them, imo it's a good idea to take advantage of them.

RE the break down on services; when you say 5 forums, 20 trust building posts and 5 site promoting threads, does that mean 4 trust building posts and 1 site promoting thread on each of the 5 forums?

I love that you provide logins and passwords. That way, customers can go in and add as many trust building posts as they please, and even add sig links, if allowed, if you won't do it for them. IMO, just finding 5 (or 10 or 20) active forums in a given niche and creating fleshed out accounts in each could be worth the price of your monthly service alone, even before you make a single post.
