I would describe a good link to be:
    - high page PR, not just domain
    - low OBL
    - dofollow
    - permanent
    - relevant content

Hard to find really really good links all the time, something usually is sacrificed

A goodlink is from a site that is relevant to your site, if it has a high pr ranking then all the better. Go for relevant links, dont go for those $2 for 10k backlink packages.

Yes, you have reason to be relevant, but has some spam filter that allows as many as it gives a bad impression and distrust.

Links from .gov and .edu sites are considered as Good links.. Then links from high PR sites, or popular sites with high traffic.

a link coming form a site which is indexed, the site must have other links pointing to it, the site must have authority in the niche, and it must be relevant to my own site in some way.

This is what i do when I search for quality links.

    Best Wishes
    Luke

Well, the links should be relevant, try to get high pr links as much as possible also don't link to spam sites, if the domain age is higher then it would give great benefit.
