Hi all i have a site (not affiliate) where i have people fill out a form so they can get information on my business . The site is about 7 years old and i was always on the first or second page when searching on some basic key words. Well my site is now nowhere to be found, no matter what keywords i use.
Is there a way to see why Google unindexed me?
All suggestions welcome.

Its not clear whether your website is deindexed or downranked. In both cases your website will not appear 'on the first or second page'.
If your site consists only of a post or page with a sign-in form you probably got slapped by Google for thin content. Google says it wants to refer people to sites that are interesting and contain useful information. If your site does not have stuff to read/see and to learn from, you're out of luck. If my guess is right, you should add lots more to your page.

If your website is hit by Goolge then you should find the cause. Find content is duplicate or not, check if your back links is coming from spam sites, Go to webmaster tool and analyze your back links. Once you found then you can change your content, remove back links from spam websites through Disavow tool, change structure of your website. These will helps you..
