[HELP] Amazon Associate - Regarding TOS

Recently amazon has changed its TOS

    https:// affiliate-program.amazon.com/gp/associates/help/operating/compare
Few day back i have started an E-Commerce website having only amazon products and added the below lines on my disclaimer page not on my all pages

You will provide us with any information that we request to verify your compliance with this Operating Agreement or any Operational Documentation. In addition to any other rights or remedies available to us, we may terminate this Operating Agreement, withhold (and you agree you are not eligible for) any advertising fees payable to you under this Operating Agreement, or both, if we determine that you or other persons that we determine are affiliated with you or acting in concert with you (whether in respect to any existing or previously terminated Associates account)
But i m confused that is it necessary to add the above lines to every page or have i done right by adding it to the disclaimer page only..

    Please Advice...

What you have listed is information for you in your capacity as an Amazon affiliate. This is not intended to be public information. Your statement of disclaimer needs to specifically state that you may make money if a visitor follows a link on your site and makes a purchase. You should go into some detail on the disclaimer page, but now days having a minimal statement on each page with product and a link to the full page is a good plan. This has become more important because now, under the current regulations, both you and Amazon can be held accountable if there is a problem.

Sorry buddy i have mentioned wrong point above i wanted to ask about

"You must, however, clearly state the following on your site: "[Insert your name] is a participant in the Amazon Services LLC Associates Program, an affiliate advertising program designed to provide a means for sites to earn advertising fees by advertising and linking to [insert the applicable site name (amazon.com, endless.com, amazonsupply.com, or myhabit.com)].""

According to this excerpt you would need the information on your disclaimer page. You would need a link to the disclaimer page on any page offering product. That would be the part of the statement within the interior quotes with the areas in brackets adjusted as necessary. The statement may look something like:
samyjain2010 is a participant in the Amazon Services LLC Associates Program, an affiliate advertising program designed to provide a means for sites to earn advertising fees by advertising and linking to amazon.com.
More cautious US bloggers add a disclaimer to any post offering products or services. They also add a statement of disclaimer to the sig on any email marketing they may do.

Means what i have done is right by putting that excerpt in my disclaimer because that page is in the footer visible on every post and page.
one more thing, Is this necessary to add amazon associate logo in the footer?

I don't recall seeing that as a requirement, but I would have to go through the new TOS to be sure. I looked it over briefly when it was released but I was largely in compliance anyway.
